I’ve put a couple of things in here for you. 1) A copy of the form as it appears to the public. Feel free to go through it and fill stuff out as much as you want. It only emails to me so you can fiddle with it as many ways as you want without breaking anything. 2) The “raw” form as it appears on the back end. The underlined blue items are the types of fields that you can use. Below that you’ll see the types of fields that I actually used and what I named them (e.g. The first field is a “text” type and I called it “name”) That allows me to do things that create check boxes and multiple options. 3) I’ve included a screenshot of all the “rules” that I created for the form…if they enter “this” then the form says “that.”
- Note: this form is divided by four page breaks that will progress you through the form while leaving items 2 & 3 below unchanged.
